Please review the information contained in the links below and make notes of any questions you may have so that we can answer them for you during your in-person orientation. Policies/Administrative Plans for Review:
The policies listed below are the most important SCLHSA policies for new employees to review. You are responsible for all of the information contained in these policies, so please read to its entirety and make notes of any questions you may have to be answered during orientation.

Health/Life Insurance Information:
You and your family members are eligible for health insurance coverage under the Office of Group Benefits. The information below gives synopsis of the plans available along with coverage rates and other pertinent information.
Other Benefits:
Other benefit programs are available through the state outside of health, life, and retirement. Please review the links below and make notes of any questions you may have to be answered during orientation.
Retirement Information:
Membership in the Louisiana State Employees’ Retirement System (LASERS) is mandatory for all state employees, except those persons specifically exempted by law. Please review the information below and make notes of any questions you may have to be answered during orientation.
Documents to be Completed:
The documents listed below MUST be printed, completed, and brought with you for submission to HR at your in-person orientation. Failure to do this may delay your start date. Please note that the W-4, L-4 and Direct Deposit Form only need to be brought with you in the event you are unable to enter the information contained in those forms into LEO. You have 30 days after your start date to make insurance elections; however, if you are sure of your elections at the time of your in-person orientation, you may enroll that day. In order to do so, please make sure to bring birth certificates and social security cards for any dependent you want to cover. Also, the Logo Embroidery form is not required at orientation, but is to accompany all garments submitted for embroidery.
